As a Guidelines Executive Committee (GEC), Task Force or Writing Group Member, you will need to complete the Declaration of Interest (DoI) Form, below.
If you are a BSH Guidelines Executive Committee and/or a Task Force Member, we must consider whether or not you have any interests that could conflict with your role. Conflicts of interest have to be declared when you are appointed; and on an annual basis thereafter.
All Writing Group Members have to declare conflicts of interest before starting work on a Guideline. This will be valid for 3 years.
The Data Protection Act 2018 – The information submitted will be held by the BSH for personnel or other reasons specified on this form and to comply with the BSH policies. This information may be held in both manual and computer form in accordance with The Data Protection Act 2018. Information may be disclosed to third parties in accordance with the Freedom of Information Act 2000. For information on how your information is retained and processed, see our Processing Notice.
NB: If you are a member of a BSH Task Force and one or more Writing Groups, you can use one Declaration of Interest (DoI) From, to denote your Task Force and Writing Group representations. Similarly, if you a member of several Writing Groups, you can use one Declaration of Interest (DoI) From submission, to denote all your Writing Group representations. You will only be sent a request for a new DoI Form, when you participate in a Writing Group, that was not denoted on a previous DoI Form.
